New Patient Registration

*Please note that the SPPG has placed a temporary freeze on registrations within the same catchment area for 3 months starting from 1st November, 2022. If you are new to the Hillsborough area or not currently registered with a GP, you can still register with us. However, if you are currently registered with another practice in this catchment area, you need to remain a patient at your current practice. 

New patients are welcome to join our practice from within our practice area which is roughly a six mile radius in some directions but not all of the health centre. This  includes parts of Moira and Lisburn but excludes Dromore and Dromara. We will need to check your exact address to confirm if you are within our boundary. 

To register, please collect the registration pack from our reception or download the forms below. Before leaving your current practice please ensure that you have enough medication for 4 weeks and have obtained your Health & Care number along with a summary of your medication lists. You will also be asked to provide photographic ID along with proof of address to submit alongside your application to join. If any children are registering who are under the age of 16, please provide a print out of their current vaccination list along with their application. If you have any questions about your registration please contact the practice and ask to speak with Gillian or Alison. Please note, that incomplete registrations may occur a delay until all the correct forms and documentation have been provided. 

Patients register with the practice, rather than with a particular doctor.

We offer all new patients a health check, and you can make an appointment when you register. The doctors would especially like to meet new patients over 65 or those with complex health needs.

Moving from outside the UK

Please complete the above form if you are moving from outside the UK. To register permanently, please complete sections 1,3 & 6 as well as providing one copy of evidence from the 3 sections on the back page. This form is from the Business Service Organisation and we must return this to them to complete your registration.

Moving within the UK

Please complete this form if you are moving from within the UK. Please submit along with a copy of your ID and proof of your address. These forms are from The Business Organisation Service. Please completed regardless of the title on them. Thank you.

TEMPORARY RESIDENTS

If you need to register as a temporary resident and have a permanent address inside NI, England, Wales or Scotland please complete the below form and submit to the practice along with a copy of your ID. Please note that we will not register patients who are already registered in another accessible GP surgery in Northern Ireland. Please ring and speak to our Office Team Leader, Gillian if you have any questions.

If you are from outside NI, England, Scotland or Wales and are from a country that has a reciprocal agreement with us, please complete the above form along with the "MOVING FROM OUTSIDE UK" from in Part 3 of the permanent registration process. You will need to complete part 1,2 & 6 in this.

When you have completed your forms, please post in the secure green post box at our front door or ring the surgery to arrange a drop off. If you drop your forms off in the secure post box, please ring any more after and ask to speak with Gillian who can check we have everything to proceed with your registration.

Please let us know if you move so that we can update your records. As we do not provide medical care for patients outside our area, please register with a local doctor if you move outside our boundary.
